2. evidence suggestions come from my own experience and may or may not be suitable for your situation... but I hope they will give you something to start from.
Element 6a: Obtain equipment needed for each activity
(suggested methods of assessment: record)

* copy of programme with equipment needed for each activity listed
* price lists from internet
* handwritten notes made at shops of material costs
* receipts for anything bought or hired
* note of where each bit of equipment is being obtained (bought, borrowed, etc.)
* note of transport arrangements for each bit of equipment
* list of equipment needed with staff or participant name by each one for who is to bring it


Element 6b: Check condition of equipment

(suggested methods of assessment: record, question, discussion)

* photos of equipment
* list of who is to check what
* checklist of equipment - tick when it's been checked
* note of any damage or defects
* copy of letter to owner of any damage of defect in equipment borrowed or hired
* copy of GA insurance cover??
* copy of unit insurance cover for any big stuff
* photocopy of any relevant safety standards (for adventurous activities etc.)

Element 6c: Brief all participants of care and the safe use of equipment
(suggested methods of assessment: question, discussion, observation)

* copy of questionnaire for participants on their knowledge
* recording or or notes on discussion on their knowledge
* copy of unit programme if training is given in use of equipment at unit meetings
* copy of information used in safety training
* copy of care instructions for equipment
* list of hazardous equipment, and storage arrangements


Element 6d: Maintain equipment on an on-going basis
(suggested methods of assessment: record, question, discussion)

* list of equipment brought to the event
* note of any repairs carried out
* photo of storage of equipment
* note of who's responsibility it is to maintain equipment
* note of any replacements required


Element 6e: Arrange for return of equipment and restocking of consumables

(suggested methods of assessment: record)

* checklist of equipment with ticks made on safe return
* copy of letter from you that went with equipment being returned to owner
* signed note from owner to say it was returned in good time and condition
* shopping list for consumables which have been used
* receipts for replaced consumables
