This agreement is made and hereby entered into, on this _______ day of_______________, ________ by and between West Virginia Sheltie Rescue, and ____________________________________, here after referred to as adopter.

I/We ___________________________________________ agree to the following terms of adoption:

• West Virginia Sheltie Rescue warrants that to the best of our knowledge, this dog :

_______ Is current on shots, is spayed or neutered, is Heartworm negative, and is taking flea and heartworm prevention.

_______ Is in good health, and free of any diseases at this time.

_______ Is in good health except for the following known medical problems:

_____________________________________________________________________________.

_______ Takes the following medication, and will need to continue to do so.

_____________________________________________________________________________.

• If, within two weeks, for any reason, you decide the dog/puppy is not right for you or your family, you may return the puppy/dog for a full refund of the adoption fee. Dog/puppy must be in the same health condition as when adopted or the treatment cost will be deducted from the refund amount given.

• Adopter understands that no one can predict what medical or temperament problems a dog may have in its lifetime. No one from our organization may guarantee anything other than the current health and temperament of this dog.

• The Adopter agrees to provide all veterinary care for this dog/puppy, that is recommended by his or her veterinarian, for the rest of this dogs life. This includes, but is not limited to: vaccinations, the treatment of parasites, fleas and ticks, and being on a heartworm preventative medication. We REQUIRE this dog to be on monthly Heartworm prevention and a flea control program throughout the dog’s entire lifetime. We recommend Sentinel. Adopter further agrees to take this dog to his/her veterinarian for a routine check up and vaccinations at least once every year. Adopter also agrees to license the dog according to any local ordinances. Adopter also agrees to provide any other medical attention the dog may need in its lifetime.

• Adopter hereby grants permission for West Virginia Sheltie Rescue to discuss this dog with my current Veterinarian. Adopter also gives his/her current Veterinarian permission to release any and all medical information to West Virginia Sheltie Rescue about this dog, or any animal that I have owned, do own, or ever will own.

• This animal is being placed with the strict understanding that it is going to a private home, to live as an indoor pet. The Adopter agrees that the dog/puppy will reside in the home of the Adopter, not outside. Adopter agrees that this dog will never be allowed to run loose or be left outdoors unattended.

• Adopter understands that West Virginia Sheltie Rescue will never place a known biter in a home with children, however, I do understand that any dog may bite if provoked. Adopter also understands that no one can predict how a dog will react to an individual child. Therefore, Adopter will use due caution when allowing any child to be near this dog, and will never allow any child to threaten, squeeze, poke, step on, or otherwise pose a perceived threat to this dog.

• The Adopter agrees that he/she is not acting as, or in part of any agent in the purchase of this Dog/puppy, and that the Adopter will not sell this Dog/puppy to any other person, agent, or pet store.

• Adopter agrees to have made, and then attach, identification tags to the dog’s collar, and make certain the dog wears it at all times. This information should include your name and phone number.

• The Adopter agrees that ownership of this dog is not transferable. The Adopter agrees that if for any reason he/she is unable to continue to house and care for the puppy/dog, or if he/she should decide that he no longer wants to keep this Dog/puppy, that he will notify and turn ownership back over to: West Virginia Sheltie Rescue.

• Adopter grants permission for a representative of WVSR to visit the home at any time to check on the wellbeing of this dog. The Adopter further agrees that West Virginia Sheltie Rescue may repossess this dog upon any evidence of neglect and/or abuse, or any breach of any part of this agreement, at no expense to the West Virginia Sheltie Rescue, and with no refund of any monies.

• This Agreement is made and signed by both parties to insure the well being and protection of this animal. If any part of this Agreement is breached, ownership of the Dog/puppy reverts immediately to the West Virginia Sheltie Rescue .

• Should West Virginia Sheltie Rescue need to seek legal action against the Adopter for any violations of this Agreement, the Adopter hereby agrees it will assume any and all attorney costs, court fees, and any other legal fee’s incurred, in the process of enforcing this contract, or repossessing the puppy/dog.
