1. List the name of all schools you have attended. For each school, include official school name, city and state of school, your major; and your date of graduation.
2. List any classes that you particularly enjoyed in school. For each class, give me official name of class, and semester and year of class.
3. List any awards you received, in shool or elsewhere. List the official name of the award or recognition, the date you received it, and the general criteria of that award.
4. List any activities that you participated in, at school or elsewhere.. Give me the official name of the activity, and the times that you were involved.
5. List the classes you found more difficult in college that you passed. For each class, give me an official name of the class, and the semester and year of that class.
6. List all of the jobs that you have had in your working career. For each job, include:
a. Name of employer b. Dates of your employment c. City and State of this employment d. Your job title here e. A generic, one sentence description of your job duties f. List four specific tasks that you performed at your job. This is where you can include unique tasks that you either performed on a regular basis, or only a few times. Include any information that makes your job sound interesting and unique.
7. List all of your volunteer work history. For each position, include:
a. Name of employer b. Dates of your employment c. City and State of this employment d. Your job title here e. A generic, one sentence description of your job duties f. List four specific tasks that you performed at your job. This is where you can include unique tasks that you either performed on a regular basis, or only a few times. Include any information that makes your job sound interesting and unique.
8. List anything you have ever written that has been published. It doesn't matter if it was published in a major print article - so long as somebody has printed it somewhere for you. Even if it was just for fun!
9. List all associations or organizations that you are, or ever were, involved with. This includes athletic teams, school organizations, local community events - you name it. List the official name of the organization or event, the location, and the general time frame in which you were involved.
10. List all areas you have ever traveled. If you have traveled extensively, give a basic summary of your major trips.
11. List all of your unique, personal interests. This is where you can put your hobbies, ranging from exercise to travel to, well, you name it!
12. List all languages that you speak - and the degree to which you speak it. It doesn't matter if you only understand a little of a foreign language - list it, and then assess your degree of fluency [or your general familiarity with the language].
13. Please state the purpose for this resume [i.e., your "objective" in your job search]
14. Include anything else about yourself that you find unique and/or interesting, and you weren't able to list elsewhere.
Remember, this is a questionnaire that will be used to put together your professional resume - so if you think that there is something in your life that would stand out to an employer - include it!!
