OK, I did the best I could with my scanner, but it did not come out too well. I hope that this is legible when you print it. If not, contact me and I'll mail you a "real" one. The form is self explanatory for the most part. The Fitzgerald Scholarship gets credit not only for money actually received, but also for pledges over a period of up to five years. Thus, if you decided to donate $1,000 over the next five years, you could do it in five annual payments of $200, and the Fitzgerald Scholarship would get credit for the $1,000 essentially immediately. THE MOST IMPORTANT THING IS THAT YOU FILL OUT THE BOX WHICH SAYS "THIS GIFT IS TO BE USED FOR_____________________", AND FILL IN "THE JOHN J. AND PATRICIA L. FITZGERALD SCHOLARSHIP". ALSO, SOMEWHERE ON THE FORM, PLEASE WRITE "ATTENTION: JOHN HANNON". No matter how much you can give, it is appreciated. It is also tax deductible, remember, as would be any contributions to Notre Dame. Remember that ANYONE can apply contributions to the Fitzgerald Scholarship, so if you have friends who are contributing to Notre Dame anyway, ask them to have the contributions applied to the Fitzgerald Scholarship, using the instructions above. |
