Need for a Troop Activity?
Please note: This page is prepared for leaders in Service Unit 155 of Girl Scouts of Northeast Texas Council. If you are from another service unit or another council, your procedures may be different. Check with your own Service Unit Manager and/or council.
Always bring the notarized GS Health History form for each adult and girl along with your First Aid kit on any outing.
When Do I Need Parent Permission Forms?
You will need a completed, signed Parent Permission Form for each girl for any activity that takes you away from your usual meeting place or time. In addition you will need completed, signed Parent Permission Forms for any activity that requires an Activity Approval Request Form. Parent Permission Forms should be brought along on the activity. It is a good idea to bring the troop copy of the Activity Approval Request Form, too.

Always check SafetyWise (the GSUSA safety manual) before making arrangements for any GS activity. If it states that council approval is needed for that activity then you need an Activity Approval Request Form. This form should be filed at council at least two weeks before the activity.
At the top lefthand corner of the Activity Approval Request Form there is a list of categories of activities that require a signed Activity Approval Request Form to be filed at council. (On the old forms there are page numbers beside each category. These are page numbers from the old SafetyWise.) The categories are:
- Simple Overnight (non-camping, camping and trips)
- Extended Trip (non-camping, camping and trips) of 3 Nights or More (take the Extended Trip Training course to find out about deadlines and additional paperwork.)
- Additional Money-earning Project
- Special Site, Equipment and/or Instructor
- Sensitive and/or Controversial Topic
- Cooperative Project with other Oganization
- Site Agreement or Transportation Charter (you will need to attach the signed agreement to the form. Only the Executive Director of the council can sign such agreements)
- High Risk Council Approval (must be sent directly to council 6 weeks before the event)
Be aware: Some activities require special equipment, extra paperwork or extra, specially-trained adults to be present (camping, swimming, boating, etc.). Check SafetyWise to be sure.
In our service unit an Activity Approval Request Form can be signed by your SU Manager, our Troop Consultant/Treasurer/Librarian, our Juliette Coordinator, or our Cookie Coordinator.
