"Time and Again"
"Basics, I" In her own cargo bay by the Kazon.
"The Q and the Grey" Captured by the Q army.
"Displaced" Abducted with the rest of the crew.
"Waking Moments" in her own cargo bay--again--but this time it's a collective dream.
"Hope and Fear" In the brig of Arturis' ship with Seven of Nine.
"Bride of Chaotica!" Trapped briefly in a forcefield on the holodeck.
"The Haunting of Deck Twelve" Barring her from the escape pods as her crew evacuates, Voyager (or, rather, the alien intelligence that has taken it over) attempts to keep its captain aboard to keep the ship running. It later traps her in a forcefield and nearly kills her.
"Shattered" First Season Janeway is abducted by that nasty Maquis rebel she's chasing...who turns out to be her future first officer.
"Workforce" Along with the majority of her crew, Janeway's memories are erased and she is put to work on the planet Quarra.
"Renaissance Man" Held hostage by the R'Kaal, who use her as a bargaining chip to get the Doctor to turn over Voyager's warp core.

Killed
"Deadlock" Janeway destroys Voyager with all hands.
"Coda" Janeway is (1) strangled by a Viidian, (2) killed in shuttle crash, and (3)euthanized against her will by Doctor.
"Before and After" Janeway died during a Krenim attack during the alternate Year of Hell.
"Worst Case Scenario" Holographic Janeway vaporized by a rigged CPR.
"Year of Hell" Janeway goes down with the ship as she flys Voyager into the Krenim time ship, successfully resetting the episode.
"Timeless" Janeway and the majority of the crew are killed when Voyager crashes -- Chakotay and Harry hit the giant Reset button.
"Barge of the Dead" Janeway is stabbed in the back by a dishonorable Klingon. Hallucination.
"Shattered" Janeway and Chakotay are dead from undisclosed causes 17 years in the future.
"Endgame" Admiral Janeway allows herself to be infected with a neurolytic pathogen and assimilated. Presumed killed in destruction of Borg transwarp hub.
