Assumption of risk and Troop 92

In the interest Of permitting Troop 92 to exist and to serve the scouting age boys of the community without fear of liability we ask you to join in this contract.  The first part is for you to acknowledge that you understand the risks involved in this activity and the second part is a release of liability.  Your signature below indicates your understanding that the terms "outdoor activity' or "activity" encompass all aspects of the activity, including preliminary and subsequent matters such as,, but not limited to,, getting outfitted for the activity, maintaining, repairing, equipment, and travel to and from the place of activity.  If, after reading this waiver you choose not to participate, please notify the Scoutmaster.

ACKNOWLEDGMENT AND ASSUMPTION OF RISK - OUTDOOR ACTIVITIES

I understand and accept that participation in outdoor activities exposes me to numerous known and unanticipated risks, which could result in personal injury, illness, death or damage to myself or my property.  Some of the risks or factors creating risk includes, but are not limited to the following:

- the hazards of travel
- swimming/ floating in unfamiliar and sometimes turbulent water;
- using paddles, ropes and other paddling and outdoor
equipment;
- hiking or walking in rugged terrain including slippery rocks;
- injuries inflicted by animals, insects, reptiles or plants;
- accidents or illness in remote places without medical facilities;
           - man-made objects in or near activities
-the forces of nature including lightning, weather changes, river level changes and others not named; - my physical condition, the physical exertion associated with hiking, camping, paddling and swimming in flowing water and other outdoor activities; - travel in a vehicle not driven by me.
-Use of fire arms
-Rock climbing
-and any and all Boy Scouts of America approved activities
I agree to accept and assume all responsibility for and risk of personal injury,, illness.. death or damage to myself or my property arising from my participation in this outdoor activity. my participation is voluntary; I choose to participate in this outdoor activity in spite of these named and other unnamed risks.  I am solely responsible for deciding to participate in this activity, and am solely responsible for deciding what equipment to take, whether to scout or run any rapid or log jam and whether to participate in any rescue or recovery of equipment.  I am solely responsible for deciding whether to participate in or continue on any river or bayou trip or other outdoor activity.

I understand my responsibility -in decision making.  I agree to obey all Troop 92 rules and regulations while participating in this outdoor activity, and I agree to obey the guidelines of the Boy Scouts of America, specifically including but not limited to, safety afloat and safe swim defense.

I have carefully read and understand this Acknowledgment and Assumption of Risk, I also understand that I will be asked to read carefully, understand and sign a separate Release of Liability.

RELEASE OF LIABILITY


I hereby acknowledge that I have read the Acknowledgment of Risk and have agreed to its terms.  I fully understand that there are certain elements of danger inherent in recreational activities, and that participating in a recreational and outdoor activity entails loss of life, personal injury, and loss of or damage to property.

I understand and agree that the terms "recreational activity," "outdoor activity" and 'activity" as used herein encompass all aspects of the activity, including preliminary and subsequent matters such as, but not limited to, getting outfitted for the activity, maintaining, repairing, loading and unloading equipment or gear, and travel to and from the place of activity.

In consideration of Troop 92, Boy Scouts of America, furnishing services to enable me to participate in this activity, I hereby voluntarily release and forever discharge Troop 92., and its Scoutmaster, Assistant Scoutmasters, Troop Committee Members and all other participating adults, both in their of ficial and individual capacities, from any and all liability or claims for any injury, illness, death or damage to myself or my property arising out of or in any way connected with my participation in this activity.  This release and discharge specifically includes, but is not limited to, liability or claims based upon the ordinary (not gross or willful) negligent acts or omissions of Troop 92 or its Scoutmaster, Assistant Scoutmasters, Troop Committee Members and all other participating adults, both in their official and individual capacities.

I further agree,, promise and covenant not to sue,, assert or otherwise maintain ' any claim against Troop 92 or its Scoutmaster, Assistant Scoutmasters, Troop 92 Committee Members and all other participating adults, both in their official and individual capacities, for any injury, illness, death or damage to myself or my property arising from or in any way connected with my participation in this activity.

in signing this document, I fully recognize that if injury,, illness, death or damage occurs to me while I-am engaged in this activity, I will have no right to make a claim or file a lawsuit against Troop 92 or its Scoutmaster,, Assistant Scoutmasters,, Troop Committee Members and all other participating adults, both in their official and individual capacities, even if they or any of them negligently cause my injury, illness, death or damage, unless that negligence is gross or willful negligence.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS.  I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND I SIGN IT OF MY OWN FREE WILL.

TROOP 92, BOY SCOUTS OF AMERICA
OUTDOOR ACTIVITY RULES OF CONDUCT AND CONSENT FORM

Since the safety and best interest of ALL our scouts is foremost.. we ask that you as parent (or guardian) review the following guidelines with your child and show your support and agreement by signing and returning it to the, Scoutmaster," of Troop 92, along with the release of liability, assumption of risk, and medical information prior to the departure date for the activity. As a participant in a Troop 92 outdoor activity you are:

required to be in your tent with lights out by 11:00 p.m. unless otherwise informed by your adult leader. - not allowed to leave the group, or the camp site by yourself unless you have permission from your adult leader.
- required to wear seat belts at all times when riding in any motor vehicle and to not stand or move around while the vehicle is in motion.
- required to use appropriate language, have good manners and respect for fellow scouts and adult leaders at all times.
- required to be responsible for your own gear, personal belongings and valuables.
- required to obey the Scout Oath, Scout Law and Outdoor Code at all times.

I, the undersigned Scout,, have read and fully understand the above rules of conduct and agree that Troop 92 reserves the right to withdraw me from the activity if I show a disregard for these rules of conduct.

As parent/ . guardian., I agree that if my child is withdrawn from the activity due to improper conduct, I will make arrangements for my child's return home immediately.  Further, I have read the description of the activity and fully understand that the program is a moderately strenuous physical activity.

In the event that my child needs medical attention while participating in the activity, consent is hereby given for any emergency hospitalization, anesthesia, operation or other medical treatment which might become necessary.
