US 491 was commissioned in 2003 to replace the remaining 194 miles of US 666. You can view photos from Monticello on this page, and photos from the endpoint in Gallup on this page.

Below is a photo showing how the Colorado DoT signed the route at first:

That was taken on CO hwy.141 near Dove Creek, approaching its junction with the dual-identity highway. Utah and New Mexico did (or are still doing) similar things with their signs; you can see examples on my Gallup and Monticello pages.
