Perhaps some or all of the PCs arrive here through a Well of Many Worlds, a gate from Sigil, a Spelljammer ship, throwing a portable hole into a bag of holding, an ancient gateway found in the astral plane, annoying the wrong wizard or a mishap exploring ancient ruins.
World hopping PCs from other places might want to help the oppressed members of their race, set up trade, just try and escape or hack out a kingdom for themselves.
A few random thoughts other people have told me about, campaign cartographer, get it or AutoREALM, use it, simple parchment looking background, use any races that interest you, its your world.
Try to include enough detail on at least one place for starting a campaign. Go easy on the front page, keep most links and crap to another page. Check out Palladium Fantasy, Warhammer Fantasy Roleplay, GURPS, Harn, Lejendary Journeys, Jorune, Thieves World, Tunnels and Trolls, Chaosium's RuneQuest and Elric for inspiration and to shake up your favorite races and monsters.
